You can make online appointment at your own convenience. Just select the “online appointment” tab and follow the instructions. It’s simple, quick and secure. If possible, please advise our staff regarding the purpose of the appointment, so that we can book you the appropriate consultation e.g. skin cancer checks, Pap smears, travel immunisations etc. We respect those who make appointments & will ensure they are seen within the allocated time. However, unforeseen events do happen occasionally and can lead to delays. We appreciate your patience whilst the un-well patients are being looked after. Should our regular patients have an urgent problem on the day, we are more than happy to see them after being triaged as urgent. Please note that an appointment is required for prescriptions repeat or referral to specialists. If you or a family member requires an interpreter service, please let us know when you make the appointment so we can organise this for you. If you are unable to keep an appointment please advise us as soon as possible (Preferably 3 Hours prior), so that we can allocate the appointment to someone else in need. ***Please Note: We are NOT a Walk-in clinic. We respect the patients that have appointments booked and strive to give them our undivided attention and highest quality medical services; please ring the practice in order to see if we have appointments available***
Notification of Cancellation is required 3 hours in advance, otherwise you will be marked as Failed to Attend. If you fail to attend on 3 occasions, you will not be allowed to make any further appointments with us, unless you pay $100, which allows us to be compensated for our loss. This policy is in place to stop any persistent misuse of our services and additionally, to ensure that there is an opportunity to reallocate the appointment. If you are late for your appointment by more than 10 min, you will have to reschedule it and you will be marked as FTA. Please Note: As long as you don’t reach your 3rd Failure to Attend by the date of our anniversary (2nd of March) your record will be cleared. ****We don’t accept appointment cancellations through email. Home and Other Visits
Home Visits are available for regular patients whose condition prevents them from attending the medical practice. Out of Pocket Fee: $100
If you wish to talk to your Doctor or Nurse, please do not hesitate to contact us during our operating hours on (03) 5281 2320. If the person you wish to speak to is not available at the time, please leave a message with the reception staff and they will contact you as soon as possible. If your call is urgent, please inform the medical receptionist.
Fees and billing arrangements
All our General Medical Consultations are Bulk-Billed (No out of Pocket Fee) for ALL patients. However there are out of pocket fees for Full Skin Check-ups; Skin Cancer & Minor Surgery Procedures; Repeated Wounds Dressings; After-Hours Consultations; Home Visits. Please refer to “Our Fees” page or ask our reception team.
Receiving the results of any test or procedure
The doctor will inform you when your investigations results will be available. It is advisable to book an appointment after that time to discuss the results with your doctor. Please ensure that all of your contact details are up to date as we may need to contact you if your result is abnormal.
In handling your medical records it is the practice policy to maintain utmost conﬁdentiality and security at all times. Your information is only available to authorised members of the staff. We follow the reformed 13 Australian Privacy principles (APPs) as set out in www.oaic.gov.au
Transfer of Medical Records
If you wish to transfer your health record to another service provider please ask your new provider to send Golden Plains Medical Centre a transfer application form signed by you. Alternatively if you would like us to obtain your records from another service provider please see reception for a transfer application form. Applications for the transfer of a health records will be processed within 30 days of receiving the request. Please Note: There is a fee for transfer of medical records of $25, in order to cover the cost involved.
